Liability Waiver & Agreement

Assumption of Risk, Waiver, and Release from Liability

I, the undersigned, am an adult entrusted to care for the child(ren) named below. I have carefully reviewed the contents of this waiver and release and knowingly and intelligently enter into this agreement, recognizing and appreciating that I am giving up mine and my child's right to sue. I am legally authorized to sign on the child's/children's behalf. I desire to allow my child(ren) and/or the child(ren) entrusted to my care to use the facilities at Little Friends Play Space. This Assumption of Risk, Waiver, and Release of Liability covers all activities, equipment, supplies, and supervision at Little Friends Play Space. I understand that any individual that is not bound by this agreement has no right to enter the facilities or participate in any activities at Little Friends Play Space.

Risk Factors: I understand and acknowledge that the use of the facilities at Little Friends Play Space involves risks including, but not limited to: bodily injury, permanent disability, paralysis, and death. These risks may result from misuse of equipment, other patrons, or lack of supervision.

Assumption of Risk: I am choosing to use the facilities at my own and my child’s risk. I acknowledge full responsibility and hold Little Friends Play Space harmless from any related responsibility.

Acknowledgement of Policies and Procedures: I agree to all policies and rules of Little Friends Play Space and understand my access can be revoked for violations.

Release, Indemnify, and Defend: I release and indemnify Little Friends Play Space and its affiliates from any losses related to property damage, injury, or illness.

Prerequisite Skills: I confirm all children in my care have the ability to safely use the facilities and that I am solely responsible for their participation.

Waiver: I waive any protections that would invalidate this agreement and agree to arbitration in Connecticut if any claim arises.

Representatives: I bind myself, my child(ren), heirs, and legal representatives to this agreement.

First Aid: If emergency First Aid is rendered, I waive any claims that may arise and this agreement remains fully in effect.
